- Frailty Team Bournemouth East Collaborative
Claire and Judy attended from the Frailty Team, they are based at Beaufort Surgery but cover all 4 practices within the network.
They mainly focus on over 65 patients and the aim is to stop hospital admissions as well as helping with social needs, signposting to the appropriate help and care planning.
This can also include taking bloods for housebound patients and administering flu vaccines. A referral would be made via your GP. - Practice News
COPD Review Clinics – Medi Sarwar is working with the network as a pharmacist, she will be holding COPD clinics to help registered COPD sufferers with ongoing issues, questions and medications etc.
A letter will be going out to invite these people to her clinics at Littledown.
MSK Service – We also welcome Neil Osbourne Director of AECC University College Chiropractic Clinic, he will be working across the 4 sites seeing people referred to him via their GP.
The aim is to help people with managing their own pain using exercises and possible treatment.
