  3. My Life My Care, BCP Council – Matt Tibbey
    Matt Tibbey came from the Commissioning and Improvement Department of the new BCP council to discuss the website My Life My Care. This is owned by the council and offers information and advice on care and support services and a directory. This resource can help with things such as lunch clubs, equipment, financial help and advice, planning for future care etc.

  4. Data Sharing
    Emma discussed in some depth the different types of sharing pf patient clinical data, what they are used for and who would have access in each case.
    Summary Care Record – standard and enriched
    This is a national scheme to provide very basic information in the case of a standard SCR such as medications and allergies and can be accessed from all over the country in a medical emergency. Additional information can be added to give more detail on your care preferences and current diagnoses. If you would like to enrich your Summary Care Record please let Emma or Reception know.
    Sharing with other Systm1 services
    This is a new function of our clinical system and allows other teams who use Systm1 in Dorset to access your medical record. This could include if you see a GP at the Urgent Treatment Centre, if you are being seen by the District Nurses or by a community dietician. There is much more information on the practice website including a list of all of the services who could potentially access your record if you are under their care.
    Sharing within the Primary Care Network
    We are starting to work more closely across our Primary Care network of four practices. Some staff already see patients from all of those practices such as the Care Team and the Clinical Pharmacist. These people will be allowed access to your record if it is relevant to your care, unless you tell us that you do not want that.
