Registering with Us

How to Register

Please bring in your medical card to the Surgery where a receptionist will help you complete it or, if you have lost your card, they will provide you with a form to fill in. You will also be asked to complete a personal health questionnaire and to make an appointment with a Practice Nurse for a basic health check. Please click on the link above to complete the Registration Form. Registrations can take up to 2 weeks to be processed, please bare this in mind if you have repeat medications, to ensure you have enough to last before you are on our system.

Visitors/holiday makers may register with the practice on a temporary basis. 

If you are a foreign visitor you may not be entitled to NHS treatment; we will treat you but you will be treated as a private patient.

Information for patients registering with the Practice

In order to make the Registration process as accurate and efficient as possible we have been advised by the PCT to obtain the following documentation for those patients who do not have a medical card:

  • A valid passport or photo driving license
  • In addition a utility bill, credit card statement or bank statement from within the previous 3 months showing your current address.

Temporary Residents

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the nearest practice to where you are staying. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Patients from other Countries

Patients from other countries can be seen for emergency treatment for urgent problems. However it is at the practice’s discretion whether to accept patients visiting from abroad for routine care.

Update your Details

If you change your name, address or telephone number, please let our receptionists know by emailing us at: [email protected] or write to us. If you move outside the practice area you will need to find a doctor in your new area.
